Mind your Allergies ........

A lot of people believe they have allergies. Its common to hear, Owh, I'm allergic to so and so and so. Thus, they avoid certain foods and things due to some experiences in the past. But how allergic are you really to the things that you believe you are? Hmmm.... Before we can answer that question. First of all, let's understand what allergies really are.



According to Wikipedia : Allergy or Allergies is defined as a damaging immune response by the body to a substance, especially pollen, fur, a particular food, or dust, to which it has become hypersensitive.

In medical terms, we describe allergies as an abnormal response of the immune system towards certain triggers termed allergens which can be anything from food, dust, mites, items in contact, heat cold et cetera, et cetera. For some reason, these allergens trigger a misguided immune response that causes physical symptoms on to a person that sometimes can be life threatening.
How I explain in to my patient is it basically like, you pinching someone and the person slaps you back. The trigger can be small, but the counter effects can be really extensive. There is a genetic link that causes allergies. Usually if a parent has an allergy, there is a 15% chance of the child having similar allergies. Some quote 30-60% chance if both parents have allergies. However, the environment still plays a major role in determining how bad your reaction would be.

The most common allergies are hay fever (which is all the seasonal flus and nasal related allergies), eczema (skin), allergic eyes (usually related to hay fever), hives (which are those wheals that form on your skin when exposed to allergen), asthma which is quite common in children and in severe cases anaphylaxis which involves severe allergic reaction that causes mucous membranes inflammation and may lead to breathing difficulties and death.

The trend of allergies seem to have increased over the years, and some believe it is due to the amount of processed diet that we are consuming daily. Certain chemicals in processed foods and colouring trigger an IgE response (immunoglobulin that reacts to allergens) thus making us more and more sensitive to certain allergens.



So how do we tackle this issue? The only way is to figure out which allergen is affecting us, and avoid it. Sounds simple? Well, not exactly. Like I always say to my patients, we do not live in a bubble. Everyday, we are exposed to pollution and the environment so there is always a chance to have an allergy. 
The mainstay of being healthy is to look after our gut. Even if you ask old Chinese traditional medicine people, or the indian Ayurvedics, they will always put a strong priority to gut health. And why is that so? In the recent years we have discovered that in adults 80% of our immune system is produced in the gut. So if the gut is not healthy, thus our immune system will not be healthy. And how do we keep the gut healthy? It all comes back to what we consume. Which is why I totally love the hair profiler epigenetics test, because it gives me an idea of how healthy a person's gut is.

But the hair profiler is just a screening device that is non-specific. So, what else can we do? In the past, we use to do skin prick Rass Test to test for allergens that may affect a person. It involves pricking the skin with the allergen and waiting for a reaction which is usually observed after a few days. However, some patients are not keen for this sort of testing that requires skin pricking and maybe uncomfortable. Luckily, with advancement of technology, we can just do a simple blood test to determine allergens. Based on which part of the world you live in, different allergens would be tested.

Don't worry too much about the weird names, your doctor will be able to explain (hopefully) what they are. And all it takes is 5 mls of blood and 5 working days and you can get your allergy profile done. The cost for the test is around RM 600 together with consultation and the blood taking service. This is mainly for external allergens taht causes immune response. There is another test called food intolerance test which tests out around 100 different food substances but that is for a blog on another day.

3 Strands of Hair is All You Need..... Hair Profiler




It is so amazing what technologies can do for us today. I came across this technology a year ago when I was doing a training at my friend's center for thread lifts. It was a semi-invasive procedure where the patient usually needed to be sedated at least to reduce pain. This was when I met the wonderful Dr Radha who introduced me to this new technology from Germany. A hair profiler using Tesla technology that can help scan the hair and read out the information contained in the hair root. 

Initially I was skeptic, but when he explained about how it was done I was excited. In the past, the hair has been used to collect information on people with chronic exposures to certain environment conditions, toxic metals and also it was already used to detect mineral deficiency. And here is a small device that is hooked to a computer and has the capability to do the same in a few minutes. The best part, the analyzed sample is then sent immediately to a server in Germany, and thus you will receive your information in 15-20 minutes.



It took us almost a year to understand the true potential of this device. There was a lot of clinical correlation that had to be used to understand what was going on with the patient. Yes, just by reading the results chart we may be able to understand the deficiencies mention and what to avoid, but sometimes the solution is not just to supplement and avoid, but a more sinister problem that needs further investigations and a few other lifestyle adjustments. You see, the question wasn't about what deficiency you have and what to take, but rather, why were you having this deficiency in the first place? Was it due to nutrition intake, or was it due to a poor absorbing bowel also termed leaky gut, or are your other systems having a pathological disease (or going towards having) that is exhausting your stores? Because of all these questions, continuous doctor training was implemented to teach the doctors how to interpret the results more accurately and save lives. 


 

For the patient, it actually gives you more information than doing a blood test. With just 3 strands of hair, we can see nutrient deficiencies for the past 3-6 months, exposures to toxic metals, radiation exposures and also other information that you will not pick up from a blood test. A blood test only gives you the now reading, except for the very few like HBA1c that shows a 3 months picture. To do a blood test for every parameter tested it would cost you a few thousand ringgit, but to do this test its only a few hundred. The main value of doing this test is to get a doctor who is able to intepret and correlate clinically based on the results given, lest it will just be another deficiency avoidance supplementation.

So instead of having to swallow so many different types of vitamins and supplements, at least you know what you really need for your body. The report also comes with some dietary advices that encourages you to take real food instead of pills.

Weight Loss Program that Works




Everyone wants to look good all the time. Everyone wants the perfect body. However, with all the different types of genetic make-ups and environmental influences, not all of us a born with that perfect silhouette. It came to the point that people take for granted the excess weight and obesity that is becoming a plague among us due to the modern sedentary lifestyle and our highly processed food diet that causes more of the unnecessary nutrients to be absorbed and stored.

I use to be really big. But growing up in a society where food was a culture, you do not realize how big you really are until you manage to lose the excess weight. Over past 4 years I have been experimenting and undergoing so many different types of diets from Atkins to blood group diet to soup diet and although a lot of them do give initial results, I do not find a long term benefit until I came across HCG diet. The HCG diet involves injecting the 125 units of the HCG hormone everyday for the period of the course and eating only 500 calories a day of only fiber and protein. There is a strict carbo restriction, strict exercise restriction and if done properly would allow fat loss without sagging.

It was not an easy protocol to follow. The first time I did it, I survived, 10 days before I felt like eating my shoes. And due to starvation, I was eating more and thus gained an additional 4 kg. Then I stopped and did more research ont he protocol and then realized, it was actually an internal manipulation of our own insulin and fat metabolism pathways. So I went through the protocol and started to follow it properly. I found that, after the 2 loading days, the HCG amount would have been sufficient to curb hunger as long as I kept my insulin level plateau down and no spikes. I also realized, you have to follow the eating times strictly initially until your brain gets the idea of the whole diet (usually after the first week) and then you can modify according to your lifestyle. But perhaps the most important component is the maintenance period where you stop injecting and go on 1000 calories of the similar diet, where the 1st stage strictly, and in the second stage starch is introduced. I also experimented on fasting and on exercise and on shorter protocols to cater busy people like me. Personally, I think that at least once in your lifetime, you should do a complete 40 day program and 80 day maintenance program in order to maintain all the weight loss and silhoutte. 

So now, after 4 years of tweaking, I guess our protocol is complete and more comprehensive. I really believe it benefits everyone who are not contraindicated to use HCG. It is perhaps the safest diet in the world as the hormone leaves the body completely after 72 hours.

Benefits of Our HCG Detox Program:

People also ask me isn't 500 calories too little to consume in a day? So if you eat more than 500, and do not expense the calories, it will restore as fats, or the metabolism will act by reducing the amount of fats burnt. Before I ever tried this diet I was ranging around 86kg. After the second one (the first failed), I dropped down to 78 but went back up to 82 because I didn't follow the maintenance properly. Then subsequently i did 40 days and dropped from 84 to 72. Again the maintenance was not followed well, but I just gained to 76. I tweaked the protocol so many times and I got smaller and smaller. The last time I did a proper HCG course was last January where from 72 I dropped down to 63 and now maintained around 65. It has been a year actually and I feel like going on again just as my yearly detox. The best part maybe was the fact that from an XL size 14, now, I wear dresses size 0-4.

Swisscelin - Revolutionary Swiss Cell Therapy to Halt Ageing.





I am back to my usual try and tested mode where I try out new products in the market and review them. Today, let's talk about Swisscelin. The first time I came across this product was when I was invited to a Hi-Tea event in Carcosa Sri Negara. It was a lovely event with beautiful setting. The principal from Hong Kong had come down for a soft launch of the product and a beautiful explanation was given by the guest Dr. speaker. Unfortunately, I was running late from my clinic appointment thus I missed out the whole talk. Such a pity.

Luckily, the marketing principle was there and she ran through with me regarding the product and the scientific evidence behind it. I'm not usually the type of person who listen much to marketing sales talks but what caught my interest was that this is perhaps the first supplement that I came across which includes the TAE or telomerase activating enzyme which was discovered in 2008 and the study published in the Journal of Lancet Oncology naming the enzyme TA-65. They won a nobel prize in 2009 and since then more and more studies have been conducted on supplementing this enzymes and its benefits and possible disbenefits. It brings a whole new ball game to the placenta cum stem cell supplementation as this enzyme has been proven to keep the DNA telomere elongated. One of the most accepted cause of ageing is when the cells fail to divide again and this has been seen related to the telomere shortening.

So of course, I was super excited when I read the ingredients. I just had to try the product for myself and see how it fairs. I stopped all supplements and medications during this experiment. There were 30 capsules per box and I religiously took a capsule a day first thing in the morning when I wake up. The capsule is quite big compared to other supplements, but because of the shape and softness, it was easy to swallow. There was a bit of a fishy smell but it was okay. I did not get any indigestion or acid reflux consuming on an empty stomach, but this is entirely on the individual. You can also take the supplement after your breakfast, although for the best absorption I would prefer eating it on an empty stomach.


So how did I fair? Day 1 to 3 was nothing much to shout about except that I had more energy in the morning compared to before. I was having fatigue issues waking up and I was blaming it on my late nights. I also realized that my sleep was deeper eventhough I slept around 5 hours a day. After the 1st week, I noticed my skin changing. It appeared softer and smoother. I also have more energy and was able to wake up fresh every morning. The pigmentation on my face improved, and I did not develop as much post inflammatory hyperpigmentation of my scars compared to previously. Sleep was great. I have enough sleep despite less hours. I even noticed an increase performance when I run every week. In terms of weight, there was no weight gain. Other things like heatiness, not as bad as taking sheep or deer placenta. I looked a lot healthier. In fact after 2 weeks, I did PRP for my face and the results was so much better than before. There was a marked reduction in pigmentation and skin glowed better. During this time, I stopped all other supplements or injections just to see how it goes. I also noticed that I do not have to take coffee first thing in the morning. I am able to delay my coffee till afternoon. I also noticed my mood swings to be less erratic and I was a lot calmer, although there was an increase of libido. But I didn't really go crazy about it. In terms of menses, I did have a early cycle that month, which lasted a little bit longer but otherwise I did not have the usual period cramps , or PMS symptoms I usually do. Even my migraine seem to have taken a holiday. After the 2nd strip I decided to try every other day. I took a capsule every other day to see if the results I obtained reversed since I've only been on it for 20 days. To my greatest pleasure, it didn't and I continued until I finished the last 10 capsules. After I finished the box, I decided to rest for awhile to see how long the effects would wear off. I only started to feel fatigued again after 10 days of completely stopping the supplement.


According to the Company's recommendation, they recommend 2 capsules daily for adults above the age of 40 for 2 months and then monthly until at least another 4 months and it will give the body enough supplement to repair itself. For younger people 1 capsule a day is sufficient unless you want to expedite the skin results. However, this supplement is not cheap and maybe realistically to maintain such expensive regime is not feasible.
Saying so, if cost was not the issue, personally I would be on this supplement for long term since I do beleive it does help to delay a bit of the ageing process the body undergoes. A friend of mine was on the supplement for 5 months and when I did her hair profiler epigenetics screening, she did not show any signs of amino acid depletion or severe aging. And she was 12 years older than me with 3 life births. She looked as fresh and beautiful as ever even without any make up on. Can't wait to start again.

It is a hot and fiery new year as we leave 2015 behind and enter 2016. Dr MM Clinic is my latest project and as of today has just reached the 3rd month of operation. After almost a year of planning and a few hiccups along the way, finally I've decided to take a leap of faith in changing the whole concept of the clinic to a more functional one.

Nepal Earthquake Experience - a life changing experience.

It has been a month since I left Kathmandu, Nepal. Coming back, my life was in such a chaos handling the work that I had left for 8 days. Exhaustion was merely an understatement as I sought the balance between my children, my work, my patients and many more decisions that required immediate attention. 1 month is perhaps a short time to resolve most issues in view of how big they were, but I am not planning to talk about all those. I want to tell you about Nepal.



My first interest in Nepal was when I was a young girl and we use to play a detective game called "Carmen Sandiago" (Yes, I am not that young anymore) - one of the cities that the thief would travel to was Kathmandu in Nepal. Nepal also hosted the highest range of mountains i.e. the Himalayas and Mount Everest. It has been a lifelong dream to see the Himalayas with my own eyes and to experience its magnificience. It is also home of many artistic cultures of Hinduism with beautiful temples and architecture marked as UNESCO heritage sites. The lovely paintings, cashmere products, beautiful literature and unique history is enough to pique my curiosity enough to agree volunteering myself for a medical relief team despite my crazy schedule.



A massive earthquake had occurred in Nepal which caused a lot of death and destruction on April 25th 2015. It was recorded at 7.8 or at peak 8.1 Richter scale Magnitude. Gorkha was hit the worst and it was estimated 8000 people died, 23,000 people injured. It was the first earthquake in more than 80 years according to the locals. Reading about it in Malaysia, I did ask my team whether are we going or not? 


So after 2 weeks they decided to send the first team and we left on the 9th of May 2015. My team consisted of 5 people with Papa Joe being the logistics coordinator, me as team leader, another male doctor and two nurses. As usual we brought medication along which luckily we had clearance from the Malaysian Nepal Embassy to bring in so there were no hiccups. It was a 4 hour flight from Kuala Lumpur but we lost 2 hours due to time zone differences. We arrived late night and by the time we went throught the visa and customs and collecting our baggage, we were out at midnight. Luckily we were welcomed by the Islamic Sangh Nepal group who facilitiated our whole stay and mission. Checking into the budget hotel provided, I was thankful that at least it had a clean bed and a proper toilet. We brought our sleeping bags just in case we need to evacuate and sleep outside.

The rendezvous point for our team was the mosque in Kathmandu. We were amongst the fourteenth or fifteenth team sent there for medical help so most of the relief works have been done. In fact most of the teams were already leaving, but it was okay. There will always be some area requiring assistance and medical attention. In my 10 years of experience vounteering for medical relief efforts, this was one of the most well coordinated effort I have ever seen. We were astounded at the amount of funds and volunteers who came in, and the amount of food and basic necessities distributed like tents and blankets. 

I spent the first day doing the official stuffs like informing the Malaysian embassy of our arrival, visiting WHO command center and getting maps to plan out our activities. We visited the health office to inform of our presence whilst my other team members followed the Pakistan group to conduct medical health checks. So I had a good look around.

Kathmandu was a beautiful city with a mix of old and new architecture. There were purple flowers growing on trees as the mark of early summer and the weather was sunny around 25 degrees day time. However, the whole city was so dusty that it was impossible to walk around without a mask. Initially I thought that it was due to the earthquake, but then I realized that they had a dry and hot weather that causes dust particles to suspend in the air longer. Coupled with on-going pollution, construction and not-so-hygienic practices it had turned this beautiful city into a dirty one. Our food choices were limited due to our own dietary restrictions but it was good enough. The mosque provides food for 500 people a day each meal time, and there were 2 shops selling Muslim food. The spices used were very different than in Malaysia, and I realized I was putting on too much weight from the lovely Biryani dishes. Another interesting dish was Momo which was something like a boiled meat dumpling and fried meatballs.


The second day we were sent to a remote area northwest of Kathmandu at the Tamang area inside district Nuwakot. Tamang are the rural folk of Nepal who live in mountains and valleys. We literally crossed 3 huge mountains, crossed a river and travelled 3 hours in a car that occasionally required us to get off and push it out of a ditch or mud. Yes, it was fun. The people were happy to see us. I met one of the UNICEF photography groups who were stationed there for a week. Pity I forgot to take their contact information. It would be nice to see their work. On the way back, it rained and thunder was clashing as we went down the mountain back to our hotel.

The third day, we were stationed nearby at a place called Sitapaila. Sita is the goddess Sita (Hindu beliefs) who was thought to be the daughter of the goddess of earth, Bhumi. She is the central female character in the Hindu epic Ramayana and was married to Rama. She is revered as the epitome for all Hindu women and is known for her devotion, self-sacrifice, courage and purity. Paila in Nepal means slipper or shoe. The village is about 10km from Kathmandu. Being in the outskirts portray more of the womenfolk who tends to the wheat fields. Whilst in the city, I could not help but notice the more male to female ratio of people walking around.

We were about to start our mobile clinic when we heard a commotion. I turned around and everyone had left the building we were in, and I heard someone shout, "Run, run, quick". We ran outside to realize an earthquake had just occurred lasting for maybe a few seconds or minutes. (I was disorientated at that time because it was my first experience).The ground shook and then it stopped. Everyone was quiet. Then in the distance I saw one building collapse, then another one, and another one. I sucked in a deep breath and said a prayer. Subsequently there were a few aftershocks not as strong as the first but no one dared go indoors. My colleague suggested we do a quick scout survey of the area to see if anyone was injured, so with 2 motorcycles we were pillions checking on the schools, and surrounding area. Luckily none were severly injured, just a few cuts and bruises. After that, we continued the clinic and we only returned to base in the evening. It was only then did we realize the magnitude of the quake. It was a 7.3 richter arising from Sindhulpalchowk in the northeast side, extending through Kathmandu, with an aftershock of 6.8 richter occurring minutes after the first (Yes, I felt both). A building in Thamel district collapsed to rubble and a few buildings cracked. At least 117 people died and 2500 people injured this time. They were more prepared due to the first episode of quake, and most people have only started to return to their homes. Subsequently, there were a few more quakes occuring on the next days we were there.



Perhaps the worst was the one that occurred at night when we were sleeping. The whole building shook and I jumped out of bed to run downstairs. It was 2 am local time. Just as we went back to our rooms, another one occurred, less strong but significant enough to make everyone hang around outside. In the morning we were informed that the quake was 6.8 richter and that Japan had an eathquake too soon after what we felt. I couldn't sleep after the quakes. I was thinking about my babies back home. How would they survive without me? I count my blessings every day that I am alive. The subsequent days we were sent nearby to Bhaktapur (south) and the last one was at Nagarkot located east of Kathmandu. Nagarkot was at a mountain district that allowed you to see the Himalayas on good days. We were lucky, we managed to see the Himalayas although Mount Everest was covered by clouds.


Experiencing the quakes was really life changing. Some people would chide that it was just tremors, but then again, what if a building toppled on top of you or you didn't realize because you were asleep? What if the tremors were signs of bigger ones to come? The Earth is changing and we are seeing more and more disasters happening. Maybe the level of pollution and ravage that we humans have done has reached a point where the Earth needs to evolve again to cope. Maybe it is the start of the end of the world.

To the people of Nepal, I enjoyed your hospitality. They were very receptive and welcoming the help we were giving. My only qualm is the cleanliness, which I hope they will improve on after this. Regardless, I will definitely come back and hopefully in much better times as a tourist, to embass in the beauty of the old city and its treasures.
